Medical Records
The Medical Record Department is responsible for the management of all patients medical records. This includes:
- Release of Information / Medical Records
- Death Certificates

Release of Information:
Release of patient information requires a signed written authorization by the patient. Copies of medical records are available upon written request with valid authorization. As established by the State of Alabama, prepayment for copies is required. $1.00 per page for the first 25 pages and .50 there after. Patient requests for medical record copies are generally available within 48 hours after the written request is received.
Death Certificates:
The State of Alabama requires hospitals to initiate the Death Certificate and submit it to the appropriate funeral home within 48 hours after the death. Funeral homes are responsible for submitting it to the State and can assist you in obtaining certified copies. The fee (as charged by the State) for a certified copy of a death certificate is $15.00. For each additional copy of the same record ordered at the same time, the fee is $6.00.
